    Team commitment form required by all team members for each weekend.

    If you feel led to work a weekend please fill out this form and we will contact you.

  4. Use this form to pay your team fees or portion of team fees using your PayPal account or Credit Card.

  5. Team Scholarship Request Form

  6. Use this form to request scholarship assistance for your team fees.  All request must be made using this form.

Reunion Group Registration Form - Use this form to register your reunion group with us so we know what groups are out available for others to join.  Even if your group is a closed group we would like to know about your group.  We won’t share your information with others unless you give us permission on this form.

  1. You can mail a check to CTTD as well for your donation and avoid the service fees that PayPal charges.  This ensures that 100% of your donation goes to CTTD.
